INTRODUCTION DATA DESCRIPTION I PROCESSING INFORMATION II CODEBOOK INFORMATION III VARIABLE DESCRIPTION LIST VI CODEBOOK 1 APPENDIX 16 I DATA DESCRIPTION The Mortality Detail File data is the source file used by the National Center for Health Statistics (NCHS) for the data reported annually in Vital Statistics of the United States, Vol. II - Mortality, from which this description is adapted. The data represent every death or fetal death registered in the 50 states and District of Columbia. The information is drawn from certificates registering each death supplied to NCHS on microfilm or magnetic tape. The Standard Certificate of Death was issued by the Public Health Service shortly before the first registration area was formed in 1880. It has been periodically revised and has been modified by each state as required by its own needs and laws. The first registration area for deaths was formed by the U.S. Bureau of the Census in 1880 and consisted of two states, Massachusetts and New Jersey, plus the District of Columbia and several large cities with efficient registration systems. The registration area had expanded to ten states by 1900, when annual collection of mortality statistics for the registration area began. One condition for admission to the registration areas was a demonstrated registration completeness of at least 90%. However, the method used in testing completeness was subject to some error and there is evidence that in some isolated areas, registation may still not be complete. The data received by the NCHS have been coded and processed by them. The classification of geographic and personal items follow procedures described in two manuals. These are the Vital Statistics Instruction Manuals, Part 3, "Classification and Encoder Instructions for Live Birth and Fetal Death Records," and Part 4, "Demographic Classification and Encoder Instructions for Death Records." Some notes in the body of the codebook have been drawn from these sources. The cause of death has been classified in accordance with the Eighth Revision International Classification of Diseases, Adapted for use in the United States (ICDA). The ICDA is based upon the Eighth Revision International Classification of Diseases (ICD) developed by the World Health Organization (WHO) and released in 1965. The codes used in these classifications take the form of NNN.n, where "NNN" is the disease classification and ".n" is a subcategory within that classification. The ICPSR has further amended this system by placing it in the form of NNN.n0 for entries with the subcategory recorded and as NNN.99 if no subcategorization is present. II This allows the elimination of blanks without disturbing the established categorization schemes. Both the ICD and the ICDA use identical three digit codes for the disease categories, and these three digit classifications are listed in the appendix. Those interested in detailed definitions and subcategory codes should consult the above mentioned sources. In the Mortality Detail File the cause of death is the underlying cause of death as assigned by the system called "Automated Classification of Medical Entities"(ACME). The system uses the manually encoded medical information from the certificates and employs decision tables in accordance with international rules to assign cause of death. In addition to the detailed classification list, recode lists, following the recommendation of WHO as modified by NCHS, are also included. FILE STRUCTURE AND PROCESSING INFORMATION The data have been examined and reformatted from the original NCHS release version. All blanks and non-numeric characters have been removed. The Archive file consists of three physical files stored on magnetic tape: a machine- readable user guide which documents the data for the human user, an OSIRIS Type 1 dictionary file which describes the data to a computer program, and a data file. This OSIRIS data file is constructed with a single 80 byte record for each case. This OSIRIS data file can be accessed directly by software packages which do not use the OSIRIS dictionary by specifying the tape locations. There are 35 variables for each of the years.
